Professional intelligence methodology
One Source. Explicit Denominators. No Facility Score.
Every published metric comes from the same canonical AHCA data and statewide report publication. This page explains exactly what is counted and what is not inferred.
Authoritative sources and coverage
Care Reliability uses public Florida Agency for Health Care Administration assisted living facility data, including FloridaHealthFinder facility and inspection records. AHCA remains the authoritative source. The current publication covers all 3,017 facilities in the canonical Florida ALF master.
Data through September 8, 2026. Last analytics refresh September 8, 2026. Data version: 73eb9cef7a51da0d619ec19bf65e596c6ddee3a95def4ba71446d71f152424ac.
Inspection and deficiency occurrence
An inspection is a dated AHCA event. A deficiency occurrence is an AHCA requirement cited in connection with an inspection. An inspection with no deficiencies remains part of the record; inspection counts are not inferred from deficiency rows.
Reporting periods
The professional views support rolling 30-day, 90-day, and 365-day periods with inclusive start and end dates. The 24-month view uses the exact shared family window, also inclusive at both boundaries. Occurrence trends use survey date, not the date Care Reliability first discovered the record.
Substantive-inspection denominator
Frequency and category-presence rates use distinct substantive inspections in the same period: Standard, Complaint, Initial Licensure, Expansion, and Change of Ownership. Monitor and follow-up activity is shown separately and excluded from this rate denominator. Standard-vs-Complaint rates use all inspections of the named type.
Counts, rates, and trends
A raw count is the number of canonical deficiency occurrences. A presence rate is the share of the stated inspection denominator containing the category or tag. A directional trend appears only when the frozen conservative Phase 6 rule passes minimum inspection, occurrence, facility, count-change, and rate-change thresholds. If the prior count is zero, percent change remains blank rather than becoming an infinite or invented percentage.
Rounding and archival source-row accounting
Displayed category shares and inspection percentages are calculated from their published integer numerator and denominator, then rounded once to one decimal. Stored archival trend rates may have four-decimal source projection precision; two-decimal ratios are presentation values. The category count of all represented inspections can differ from the count of substantive inspections containing it.
The September archival totals preserve retained AHCA source rows, including zero-tag INITIAL COMMENTS entries. Those administrative entries are shown separately and must not be read as cited deficiencies. Unmapped requirements remain unavailable for substantive qualification; the preserved totals are not a qualified clinical-deficiency count. Monitor/follow-up activity is excluded from both the numerator and denominator of substantive-inspection rates.
Standard and Complaint surveys
The comparison reports observed inspection counts, deficiency-bearing inspection counts, deficiency occurrences, and rates. It does not imply that one survey type or the facilities in it are more dangerous.
Recurrence
Recurrence requires the same facility and AHCA tag or Care Reliability category in more than one distinct inspection during the defined window. Repeated occurrences inside one inspection do not count as recurrence. Prevalence uses facilities with at least one finding in that same category or tag as the denominator.
Co-occurrence
Co-occurrence means two categories appeared in the same inspection. Each pair is counted once, requires minimum inspection and facility volume, and excludes self-pairs. Co-occurrence does not establish causation.
Taxonomy and unmapped findings
Care Reliability groups AHCA tags into provisional categories and process domains for analysis. These are organizational labels, not AHCA's official classification system. The raw AHCA tag and requirement remain authoritative. Unmapped source requirements are retained in the archival totals and disclosed separately from known initial-comment entries; unreviewed new tags are not given invented process labels.
AHCA deficiency classes
These summaries paraphrase Florida Statutes, section 408.813. Florida's assisted living facility enforcement statute refers to those classifications; see Florida Statutes, section 429.19. They are AHCA classifications, not Care Reliability ratings.
- Class I
- AHCA determined that the violation presents imminent danger or a substantial probability of death or serious physical or emotional harm.
- Class II
- A violation, other than Class I, that AHCA determined directly threatens a resident's physical or emotional health, safety, or security.
- Class III
- A violation, other than Class I or II, that AHCA determined indirectly or potentially threatens a resident's physical or emotional health, safety, or security.
- Class IV
- A violation involving required operational or documentation conditions that AHCA determined does not threaten resident health, safety, or security and has no potential for a negative effect.
- Unclassified
- AHCA may cite certain authorized violations without designating them Class I through IV. Care Reliability preserves that source classification.
- Unknown
- The canonical source record does not provide a class value that can be placed in the listed AHCA classes.
Correction dates and data freshness
A correction date is shown only when it appears in the canonical AHCA deficiency record. The interval is days between survey and AHCA-listed correction date; it is not labeled days out of compliance. Missing dates are not called uncorrected. Newly conducted activity uses survey date; any future newly detected feed will use source-first-seen date. Those concepts are not mixed.
Regulatory Change Watch
The watcher checks 9 authoritative Florida sources: AHCA's assisted-living, rulemaking, current ASPEN survey-regulation, and survey-form pages; Florida Administrative Code chapters 59A-35 and 59A-36; Florida Statutes chapters 408 Part II and 429 Part I; and the Florida Administrative Register's recent-issue source for applicable AHCA/ALF notices.
Each retrieval retains an exact source hash and a normalized hash. Normalization removes navigation, footers, transient session values, analytics markup, and whitespace churn while retaining rule text, status, dates, links, document versions, form revisions, and hearing or workshop notices. Only a substantive normalized difference proceeds to object-level comparison.
The initial run establishes a baseline; it does not present every current rule or form as a new change. Later records preserve developmental, proposed, adopted, effective, amended, superseded, and repealed states. Proposed material is not described as binding, and no effective date is inferred when the source does not publish one.
Materiality labels separate source formatting changes from changes that may affect ALF operations. Ambiguous or interpretive changes require human review. Only reviewed, valid or revised material and potentially material records can be published. A source failure is shown as an availability problem and can never be converted into a no-change statement.
Potentially affected process domains are organizational pointers, not facility-specific compliance conclusions or instructions to rewrite a policy. Current regulatory-watch data version: 8b802446b3b1d7e064d5663499571649714d16e4bd899fc2b822ac8d82773f13.
What Care Reliability does not infer
This intelligence does not determine a facility's internal root cause, present compliance, quality, safety, resident risk, or future performance. It does not provide a score, grade, legal opinion, regulatory representation, or official AHCA service. Facility and county leaderboards are not produced.
