No public health-information submission
Do not place a resident name, date of birth, diagnosis, medication list, medical record, photograph, discharge document, or other protected or identifying health information into the urgent AHCA inquiry form or an ordinary email. The facility and family contact pages do not request case details through a questionnaire.
Authorization and access
Record review and facility access require valid resident or representative authorization and remain subject to applicable privacy, resident-rights, safety, and facility procedures.
Secure transfer is arranged separately
When authorized records are required for a defined engagement, Care Reliability Partners provides separate secure-transfer instructions. The absence of a public upload field is intentional.
Independent family review
Family reviews are independently retained by the resident or family and do not make Care Reliability Partners an employee, agent, surveyor, or representative of the facility.
Separate facility work
Care Reliability Partners keeps facility consulting and family reviews separate. Confidential information from one engagement is not used in another. Work on both sides of the same resident, event, or active facility matter is not accepted unless it has been appropriately disclosed and authorized.
No status claim
This notice describes the website and service boundaries. It does not state that Care Reliability Partners is a HIPAA covered entity, business associate, or HIPAA-certified service.
