Dashboard access requests
When you request free dashboard access, we retain your name, work email, organization, role, facility details and optional message to review your request and contact you about access. Requests are stored in our protected administrative system. Submission does not create an account or subscribe you to a newsletter. Do not include resident-identifying or medical information. Contact contact@carereliability.com to ask about correction or deletion.
Newsletter and measurement
Newsletter subscription requires affirmative consent and email confirmation. Resend manages delivery and subscription preferences. We retain consent evidence, source attribution, delivery events, and suppression evidence. Public business contacts used for a one-time manual introduction are not newsletter subscribers.
First-party session storage preserves sanitized campaign parameters and opaque outreach tokens for up to 24 hours. We record page visits and visible-page engagement, distinguish likely automated scanners, and measure newsletter conversions. We do not use fingerprinting or collect resident information. Email open signals may be generated by privacy or security services and do not prove that a person read a message.
Visit details are retained for 90 days and delivery details for 12 months. Consent evidence is retained while subscribed and for three years afterward. Minimal suppression evidence is retained to honor unsubscribe and no-contact requests. Contact contact@carereliability.com for privacy requests.
Public-site scope
This website is for general information, service scope, free tools, pricing, and initial inquiries. It is not a patient portal, medical-record system, emergency service, or public records-transfer service.
Information the public website requests
The facility and family contact pages do not use questionnaires. The separate urgent AHCA form requests contact information, an organization, optional citation and date information, and a short general description of the response need. The voluntary ALF Intelligence subscription form requires an email address and may accept a first name, professional role, facility, county, and selected email topics.
The complimentary process-map form requests business contact information, facility or operator name, role, selected process area, and a brief general workflow description. Optional fields include phone, county, and preferred contact method.
Information not to submit
Do not enter a resident name, date of birth, diagnosis, medication list, medical record, photograph, discharge document, or other protected or identifying health information. Do not upload records through the public site. Authorized secure-transfer instructions are arranged separately when needed.
How contact and inquiry information is used
Contact and urgent-inquiry information is used to understand the general request and respond to the person who submitted it. Resident-specific information obtained during a family engagement is not used in a facility consulting engagement.
Process-map request information is used to review scope, reconcile the named facility when an exact authoritative match is available, manage the requested map, and record manual follow-up or conversion stages. If the public form detects apparent resident-identifying information, the description is not retained as submitted; it is replaced with a review flag and redaction notice.
Subscriptions and email delivery
Subscription information is used to deliver only the selected Care Reliability updates. Resend is the email delivery provider and may process contact, delivery, bounce, complaint, unsubscribe, and link-click events needed to operate and reconcile the mailing list. Report and process-map links are identified separately so Care Reliability can measure those email actions. An unsubscribe request, suppression, or bounce prevents future sends under the applicable status.
Separate engagements
Care Reliability Partners keeps facility consulting and family reviews separate. Confidential information from one engagement is not used in another. Work on both sides of the same resident, event, or active facility matter is not accepted unless it has been appropriately disclosed and authorized.
Analytics and cookies
This website installs no external site analytics and uses no nonessential cookies. The process-map workflow records first-party CTA impressions, clicks, form starts, and completed requests with a random session-scoped attribution identifier. It does not use cross-site tracking. Email delivery events described above are limited to operating the voluntary distribution program.
Questions or requests
Use the Contact page and do not include resident-identifying or medical information in ordinary email.
