Activity observed since tracking began — September 11, 2026
Status: Verified activity with coverage limitations
Reporting interval: after September 5, 2026 at 5:30 PM EDT through September 11, 2026 at 11:02 AM EDT. This is an acquisition-date window. Inspection dates may be older. Retained inspection analysis is through September 8, 2026.
Coverage: 3,017 of 3,018 facilities have complete history. Pending history: TRANQUIL OAKS AT MONTEVISTA. These are observed records, not an estimate of missing findings or complete statewide activity. This corrected edition uses the same source cutoff as the original issue; the original remains in the archive.
- Collection is incomplete or the latest collection was unsuccessful. Counts show verified records observed so far, not complete statewide totals. Missing records are not treated as zero.
- The post-backfill acquisition baseline does not yet cover 30 complete days.

Methodology and limitations
Method CR_WEEKLY_ACQUIRED_V2. First-seen timestamps after the post-backfill baseline (September 5, 2026 at 5:30 PM EDT) identify acquisition, not AHCA publication or the date a deficiency occurred. Intervals exclude the start and include the cutoff. Records are deduplicated by canonical identifiers; only currently observed, matched deficiencies and current inspections are eligible.
A seven-day ranking requires at least 20 findings across 10 facilities and seven complete days of post-backfill observation. Otherwise this report switches to 30 days and applies the same minimums plus 30 days of observation. Incomplete collection still permits observed record counts and descriptive categories, with explicit coverage limits; full-period rankings remain unavailable. These conservative editorial thresholds do not establish statistical significance. Four-week comparison requires 35 days of comparable history and successful cycles. No facility is labeled an outlier.
Each issue is frozen to its cutoff and archived. Later data does not rewrite it. A held issue documents a reporting limitation; a later successful issue does not retroactively complete this one.
