Staffing: verify the handoff when coverage changes
What changed → why it matters → what to check → how to get help.
Educational draft — clinical and regulatory review pending · Revision 2026-09-11 · Version 1.0
What AHCA commonly cites
Staffing is among the leading categories in the canonical 30-day sample. Public findings do not establish today’s staffing adequacy.
Why the process commonly fails
The roster may change without transferring responsibility for unfinished work. A listed role is not evidence that the receiving person understood the task. This may indicate a control gap and should prompt verification; it does not diagnose a facility.
What to verify today
- Trace one recent call-out or shift reassignment.
- Check who accepted each time-sensitive responsibility and where that handoff was recorded.
- Verify the escalation route when qualified coverage cannot be confirmed.
What evidence should exist
The approved coverage plan, actual assignment, acknowledgment, qualification checks as applicable, and documented resolution of gaps.
One sign the issue may be systemic
Open tasks repeatedly have no acknowledged owner after shift changes.
